Javascript on custom page.

I need to run some javascript on page load on a custom page. I tried putting a script tag on my custom page, but when I inspect the page as it appears on the user-facing website, my code isn't present. Furthermore it looks like several other elements are stripped out, including a form with two inputs and some CSS styling.
My code should give you a solid idea of what I am trying to accomplish:
